Sec. 5-62. Application for non-emergency medical transportation carrier certificate.
An application for a non-emergency medical transportation carrier certificate shall be made upon such form as
may be prepared or prescribed by the City Manager or his designee, and shall include:
(a) The name and address of the applicant.
(b) The trade or other fictitious name, if any, under which the applicant will do business.
(c) The location and descriptions of the place or places from which the applicant intends to operate.
(d) A copy of the applicant’s license from the Virginia Office of Emergency Medical Services showing that the
applicant has been properly certified as an emergency medical services agency.
(e) A list of the vehicles the applicant plans to use to provide non-emergency medical transportation services
and a copy of the permits showing that such vehicles have been approved for non-emergency medical
(f) A copy of a certificate of insurance showing that the applicant has obtained the insurance coverage required
by section 5- 64 of this article and that such insurance is in full force and effect.
(g) Such other information as the city manager or his designee shall deem reasonably necessary to determine
an applicant is qualified to provide non-emergency medical transportation services.
(h) Once all of the required information and documents are submitted to the City Manager or his designee, the
City Manager or his designee shall have thirty days to review such information and documents to determine if
they meet the requirements of this section, and to issue or deny the certificate. If the City Manager or his
designee, denies an application for a certificate, the City Manager or his designee, will send the applicant a
notice of his decision, stating the reasons for the denial of the certificate. (Ord. No. O-14-019, 03-11-14)
